To make your first appointment as easy as possible, we have prepared the following preparation checklist. If you have any questions about any of these items or your appointment, please feel free to call or email us. Here are the things you will need for your first appointment:
X-rays:
Your family dentist may have sent your x-rays to us in advance or given them to you to bring to our office. We may still need to take other x-rays to complete our thorough examination, but it is always our preference to use your recent x-rays.
Insurance Information:
If you have dental insurance, it is helpful if you bring your insurance card and be familiar with your dental benefit plan.
Health History Form:
Rather than coming to your appointment 15 minutes early, you can complete our Health History form at your convenience and print at home. Simply fill out the form and click “Print Form”. You can then either fax the completed form to us at (847) 362-7932 or bring it with you to your first appointment.
